Please print first & last names as shown on a valid passport, drivers license, FHF or USFHA  membership
card.
Date of birth for all players in the "Under Divisions"
must be listed. This will be used for player verification &
eligibility purposes for Under Divisions.
EACH PLAYER MUST HAVE A DESIGNATED NUMBER THAT IS VISIBLE (IDENTIFIABLE) TO BOTH
UMPIRES FROM ANYWHERE ON THE FIELD.
A maximum of 16 players may be registered except in under 14, under 12 and under 9 divisions in which a
maximum of 11 will be accepted.
A minimum of 8 players must be listed for all divisions in order to enter a team. Names entered may be
changed or deleted by submitting a new form or during registration night.
California Cup does not provide medical insurance for participants. All players must have medical
insurance which is valid in the United States of America.
All players under the age of 18 must complete the Emergency Information & Consent to
Treat Form.

  • If a team's designated umpire does not register online by the deadline of May1st, you
    will forfeit your Umpire's Deposit.
By submitting this form, it does not guarantee your team's acceptance into the tournament until a check
(non-refundable) has been received by April 1, 2009 and cleared by the bank. Please mail a check for
